Partnership HealthPlan Contracted Provider
As a contracted CalAIM provider, our housing and support services are covered for eligible Partnership HealthPlan and Medi-Cal members—at no cost.
We serve participants throughout Solano County (Vallejo, Fairfield, Vacaville) and Sonoma County (Santa Rosa) with wraparound services addressing medical, behavioral, and social needs.
