What You'll Need
Before starting your referral, please have:
Client Information - Name, DOB, Medi-Cal ID
Clinical Documentation - Discharge summary, H&P, or recent clinical notes
Coverage Details - Partnership HealthPlan member ID or Medi-Cal verification
Contact Information - Your direct line for coordination

We respond to all referrals within 1-2 business days. For accepted referrals with available space, admission can occur immediately upon discharge.
We'll review their needs and recommend the appropriate level of care from our different service options. One referral gives you access to our full continuum.
Yes. All referral processes and communications meet HIPAA security requirements.
We'll provide estimated availability and can maintain a priority waitlist. We'll also suggest alternative placements if appropriate.
Hospital discharge planners, case managers, social workers, behavioral health providers, Partnership HealthPlan care coordinators, probation officers, and other healthcare professionals.
No cost to patients or referring agencies. All services are covered for eligible CalAIM members through Partnership HealthPlan. We handle authorization directly.
