Transitional Housing in Solano & Sonoma Counties
Your bridge to independent living—structured support in Northern California as you build skills and stability for your future
The path from crisis to independence isn’t always straightforward, and you shouldn’t have to walk it alone. Hammock Homes’ transitional housing programs provide more than temporary shelter—we offer structured environments where you develop the skills, confidence, and resources needed for lasting stability.
What Makes Transitional Housing Different
Transitional housing sits between emergency shelter and permanent housing, offering time and support to prepare for independent living. Unlike shelters focused on immediate crisis, our supportive transitional housing emphasizes skill development, employment readiness, and building the foundation for long-term success. Think of it as an intentional pause—a chance to catch your breath, heal, learn, and plan while surrounded by support. You're not rushed out the door you're given the time and tools to leave ready.
Who Thrives in Transitional Housing
Our programs serve individuals at critical life transitions who need structured support:
Completing substance use disorder treatment and maintaining recovery
Transitioning from psychiatric hospitalization to community living
Returning to the community after incarceration
Aging out of foster care and learning to navigate adulthood independently

Moving out of chronic housing instability into permanent housing
Anyone committed to personal growth and achieving housing stability
