Transitional Housing: From Care to Independence
Extended stability and skill-building for lasting independence
When clients need more than temporary housing but aren’t ready for independent living, Hammock Homes provides structured transitional housing in Solano & Sonoma counties. Our programs bridge the gap between crisis and self-sufficiency for CalAIM members across Northern California.
